PT Pos Indonesia (Persero) is a Public Company which is engaged in courier services both in the form of letters, packages, or money. One of the delivery service offered by PT Pos Indonesia is the Pos Express. Pos Express is a one-day delivery service through which the shipment will arrive at the destination address one day after the submission through the process of collecting. In this research, which became the object is Processing Centre Semarang Office. From the monitoring results through the application of Tools Monitoring Operasi, it appears that Standard Time Pos Express service for outgoing in Region VI Semarang never reach the target of 95%, especially in the months of January to March 2016. To find out the cause of failure to achieve this SWP, analysis method like FMEA ( failure Modes and Effects Analysis) id needed. FMEA is a method through three parameters: assessment of severity, occurance, and detection. FMEA can be seen with the three modes of failure are worth the highest risk by calculating the Risk Priority Number (RPN). Used Analysis of Barriers (Barrier Analysis) to analyze the barrier of the failure modes that have been prioritized so that it can formulate recommendations to strengthen the barriers that are expected to reduce the risk of failure modes.
